Call the NDIS. Reach them on 1800 800 110 and request an Access Request Form. They will ask you some initial questions before sending the form to you via email or post.
Download the Form. Download the Access Request Form from the NDIS website. Fill it out and email it to nat@ndis.gov.au.
Apply with Help. Ask the help of a local area coordinator (LAC), Early Childhood Partner, or your GP/specialist when applying for NDIS. They can help you gather the documents you need and complete the form.
That said, you need to provide the following:
o Proof of identity
o Supporting evidence of your disability
o Consent to share information
